There are generally accepted aesthetic rules, of course; however, it is the recognition of the specificity of each case that will lead to the selection of those techniques that will guarantee an absolutely natural result that will meet the expectations of the patient.

The use of the Piezo ultrasound device was introduced in 2013 in modern rhinoplasty as a new revolutionary method. The most important advantages of this technique are that it ensures greater precision in surgical manipulations while protecting the soft tissues from injury at the same time. The Piezo device exploits the piezoelectric effect based on which it achieves the rapid pulse motion of special metal heads at a frequency of 25 to 29 kHZ. What is the appropriate age?

A prerequisite for someone to undergo Rhinoplasty is that their facial features -and therefore their nose- are fully developed. In women this happens at the age of 16-17 years and in men at the age of 18-19 years. As for older ages, there is no cap limit, so any person who feels the need to improve the appearance and function of their nose can undergo Rhinoplasty surgery.

Is there a suitable season for Rhinoplasty?

No. The operation can deliver the same good results and be followed by an equally rapid recovery regardless of the season in which it is performed. Of course, in any case it is necessary that the patient follow the post-operative instructions. Thus, the date of the operation practically depends on the schedule and the time frames of the patient and the surgeon.

What is the procedure of the operation?

Rhinoplasty is performed in organized nursing centers. Before the operation, a pre-operative check-up (blood tests, evaluation by a cardiologist and an anesthesiologist) is necessary, which in most cases is carried out on the day of the surgery, a few hours before the operation. The patient is then transferred to the operating room where the procedure takes place under general anesthesia. They are then taken to their room where they stay for at least 6 hours. After this period, and depending on their wish, they can return to their home or, alternatively, they can spend the night in the clinic and be discharged the next day.

Can my breathing issues be corrected at the same time?

Certainly. Along with the aesthetic problem of the nose, the experienced and trained Nose Surgeon can correct any breathing issue in the same operation, by straightening a crooked nasal septum, executing reduction of the hypertrophic nasal turbinates and doing endoscopic removal of nasal polyps.

Rhinoplasty is a unique operation for each patient. Nevertheless, it is governed by some generally accepted basic aesthetic rules, the application of which, to the extent possible, can contribute decisively to an excellent aesthetic result. In this light, it is advisable to highlight some differences, in terms of the desired result, for women and men who undergo rhinoplasty, without of course, these differences always being an inviolable rule. Thus, the nasolabial angle, that is, the angle formed between the nose and the upper lip, in women should ideally be between 95 and 110 degrees, while in men it should be between 90 and 95 degrees. In other words, for women, a slightly more “upward rotated” tip (but on no occasion an excessively rotated tip that refers to a “French” nose) is aesthetically acceptable and can add beauty and refinement to the whole face. Especially for people of shorter height, this feature is considered more suitable. On the contrary, the aim for men is an almost right (90 degrees) angle that matches the rest of their masculine features. By analogy, the aim for women is for the bridge of the nose to be slightly lower than that of men, which makes the tip stand out subtly, giving the nose a more feminine profile. On the contrary, the aim for men is for a bridge that is almost at the same height as the tip and gives the image of a “strong” male profile. In any case, modern rhinoplasty includes a multitude of available techniques, the application of which allows us to ensure an aesthetic result that will meet the preferences and expectations of each individual patient. 

What are the most common nasal aesthetic problems that are corrected with rhinoplasty?

 

Modern rhinoplasty offers exceptional capabilities, making it possible to simultaneously correct a range of anatomical and aesthetic issues of the nose. This allows us to achieve an excellent result that is in full harmony with the rest of the facial features.

Dorsal Hump, the well-known bump on the bridge of the nose.

In the vast majority of cases, it consists of both bone and cartilage. Its removal during rhinoplasty can significantly enhance the nasal profile, often transforming the overall appearance of the entire face.

Bulbous/Wide Nasal Tip

This is one of the most common aesthetic concerns among patients seeking rhinoplasty. It is typically caused by overgrowth of the lower lateral cartilages, which shape the tip of the nose. In some cases, thick skin in the nasal tip area also contributes significantly to the problem.

Treatment involves the careful and controlled removal of portions of cartilage, followed by reshaping using sutures. This approach helps refine the nasal tip and brings it into better harmony with the other anatomical structures of the nose, creating a smooth and natural continuation of the nasal dorsum rather than an element that looks disproportionate.

Drooping Nasal Tip

Ideally, the nasolabial angle, the angle between the nose and the upper lip, ranges from 95° to 110° in women and 90° to 95° in men. When this angle falls below 90°, we refer to it as a drooping nasal tip.

In addition to its aesthetic impact, a drooping tip can also cause nasal breathing difficulties, as it may obstruct airflow through the nasal passages. Correction involves specialized techniques that allow us to reposition and support the nasal tip cartilages, ensuring they remain in their new, elevated position permanently.

Overprojected Nasal Tip

When the nasal tip projects excessively forward, it creates a significant aesthetic concern that must be addressed during rhinoplasty.

The surgical approach typically involves reducing the length of the cartilages that shape the tip. In some cases, it is also necessary to reduce the size of the anterior nasal spine, a bony structure that supports the tip via the septum. These techniques help restore facial balance and create a more natural nasal contour.

Underprojected Nasal Tip

In this case, the nasal tip lacks adequate projection, giving the appearance of a small nose that is disproportionate to the other facial features or the patient’s overall facial structure.

Correction involves specialized manipulation of the nasal tip cartilages. In many cases, cartilage grafts, taken from other parts of the nose, are used to “build” a new, more defined and projected tip, enhancing balance and harmony with the rest of the face.

Crooked/Deviated Nose

When the nose visibly deviates to one side, it causes a significant aesthetic concern for both women and men.

In such cases, it is usually necessary to simultaneously straighten both the nasal septum and the nasal pyramid using osteotomies (controlled bone cuts). Additionally, special cartilage grafts are often used to support and maintain the corrected alignment, ensuring both functional and aesthetic improvement.

Nose after Trauma or a Previous Failed Surgery

In both cases, one or more anatomical structures of the nose have been significantly damaged. A common deformity is the saddle nose, in which the nasal support is lost—either due to trauma or excessive cartilage removal during surgery.

Other frequent anatomical issues include asymmetry of the nasal bones, irregularities along the nasal dorsum, tip deformities, and septal deviations.

Treating such cases requires specialized training, extensive experience, and surgical precision, as complex techniques are often necessary to achieve complete nasal reconstruction and restore both form and function.

How long does the procedure take?

Rhinoplasty is a delicate and highly demanding surgical procedure. The primary goal is to achieve the best possible result, regardless of how long it may take. On average, the operation lasts between 1 and 3 hours, depending on the extent of the functional and aesthetic issues that need to be addressed.

What happens after the procedure?

The patient is typically discharged 6 to 8 hours after the surgery or the following day. A small nasal splint is usually placed and is removed on the 7th postoperative day at the clinic, along with the columellar sutures in the case of an open rhinoplasty. A short course of antibiotics is prescribed, and nasal rinses with a special solution are recommended for several days.

Does the operation hurt?

Rhinoplasty is a painless surgical procedure. General anesthesia ensures the absence of pain during the operation, and after surgery, most patients require little to no pain medication.

When can I go back to work?

Following rhinoplasty, we usually recommend one week of relative rest. Any postoperative swelling starts to subside significantly around the third day, allowing the patient to resume light activities.
By day 10, it’s unlikely that anyone will notice you’ve had surgery.

  • Ask to see a wide range of before-and-after photos of actual patients. These photos should not only be from the immediate postoperative period, but request to see results after 6 months, 1 year, or more. Also, make sure the photos show the nose from multiple angles (profile, front view, ¾).

Open or closed functional rhinoplasty?
Functional rhinoplasty can be done with either the closed or the open approach. Depending on the case, the two methods can offer excellent results both aesthetically and functionally. But it is an indisputable fact that open rhinoplasty is the most appropriate for the most demanding cases. The reason lies in the fact that it provides a clearly better surgical field than the closed one. It thus allows more and finer surgical manipulations, while at the same time provides the possibility for easier retrieval and more precise placement of grafts.

In closed rhinoplasty, the surgeon makes small incisions inside the nose, through which they gain access to all the anatomical structures that need correction.

It is a reliable and well-established technique that can deliver excellent both aesthetic and functional results, without leaving any visible external scars.

In open rhinoplasty, access is achieved through an incision made at the base of the columella, which extends into the inner part of the nostrils. This approach allows the surgeon broad and direct visibility of all nasal structures, enabling a detailed and accurate assessment of the entire area.

The surgical field is significantly wider than in closed rhinoplasty, making it possible to evaluate and modify all anatomical components while keeping them in their natural position, without pulling or distortion.

At the same time, open rhinoplasty allows for easier placement and more secure fixation of cartilage grafts when needed. The incision at the columella is closed meticulously with very fine sutures, and within three to four months, it typically becomes invisible or barely noticeable, posing no aesthetic concern for the patient.
